Aqueous Flow Measurements in Man by the Perilimbal Suction Cup Technique. Ii. Comparisons in Chronic Simple Glaucoma before and after Pilocarpine.

چکیده
WITH the observation by Laqueur (1876) that tension of glaucomatous eyes could be reduced with physostigmine, and the corroboration of Weber (1877) that the same effect attended the use of pilocarpine, both substances have found a permanent place in therapy. Their precise action, however, remains obscure (Sugar, 1957; Becker and Shaffer, 1961). Unlike carbonic anhydrase inhibitors, such as acetazolamide, which lower the intra-ocular pressure in glaucoma by reducing the rate of aqueous formation without alteration of the outflow facility, pilocarpine relieves the obstructed outflow of aqueous in glaucoma. This effect might be explained either by a direct action upon the aqueous outflow episcleral veins or by a direct reduction in the rate of aqueous formation. The suction cup technique, introduced by Ericson (1958) provides measurements of:

Venous pressure opposing aqueous outflow in patients with and without chronic open-angle glaucoma. Calculation from data obtained by indentation tonography and suction cup analysis using an electrical model of the eye.
Three groups of patients (those with chronic open-angle glaucoma, glaucoma suspects, and normals) were subjected to both tonography and suction cup analysis. The data, analyzed by an electrical model of the eye, showed that (1) venous pressure opposing aqueous outflow is higher in patients with chronic open-angle glaucoma and in glaucoma suspects than in normal patients; and (2) lack of pressur...

OCCLUSION of the anterior chamber outflow pathways by means of a perilimbally placed suction cup leads to an obvious rise in intra-ocular pressure (Rosengren, 1934, 1956). From this increment in intra-ocular pressure, various investigators (Ericson, 1958; Linner, 1959; Weekers, Prijot, Feron, and Verrier, 1960; Galin, Baras, and Mandell, 1961; Langham, 1962) have estimated the rate of aqueous f...

IN the past few years there has been increasing interest in the use of the pressure cup technique for the investigation of aqueous humour dynamics. Rosengren (1956) demonstrated a rise of intra-ocular pressure when a positive pressure was applied to the sclera overlying Schlemm's canal. He suggested that this rise of pressure was due to continued formation and accumulation of aqueous humour, th...
متن کاملIntraocular pressure reduction in chronic simple glaucoma by continuous infusion of dilute pilocarpine solution.
Ten hospital outpatients with bilateral chronic simple glaucoma received a single drop of 2% pilocarpine to one eye and a continuous infusion of 0.1 % pilocarpine at a flow rate of 0.01 ml/min to the other eye (both solutions were at pH 7.2). On another occasion the treatments were reversed. Measurements of intraocular pressure (IOP) and pupil diameter were made at 30 min intervals for 2 hours....
متن کاملSympathetic control of aqueous secretion in man.
The use of adrenaline in the treatment ofopen-angle glaucoma has both clinical acceptance and experimental support. But the mechanism underlying its ocular hypotensive action is still poorly understood. The role of the sympathetic nervous system in the regulation of aqueous dynamics is also not clearly worked out for man, though our understanding has been greatly increased in recent years. It i...
